We are affiliated with Catholic Medical Partners, a network of physicians developing a new standard for health care, one in which the patient is an active and educated participant.
We have implemented a new program called care coordination to help enable patient take control of their health. With this program, we plan to help our patients increase awareness and knowledge of their disease, improve their quality of life and decrease hospitalizations.
Things we may be doing for you include making phone calls providing educational materials about your health, sending or calling you about important test or consultations that need to be done, meeting with you in the office, and showing you how to do the best job you can to help yourself stay healthy.
